Clear aligners seem almost too simple. You wear a thin piece of clear plastic over your teeth, swap it for another one every two weeks, and somehow your smile transforms. So how does it actually work — and what are the limits? Here's the straight answer, explained by the people who design these treatments every day.
The basics of tooth movement
Your teeth aren't bolted into your jaw. They sit in sockets surrounded by a thin, elastic tissue called the periodontal ligament. When you apply consistent, gentle pressure in one direction, the bone on the pressure side gradually reabsorbs, while new bone forms on the opposite side. Your tooth literally moves through the jawbone.
This is the same biology behind every orthodontic treatment ever invented — from old-school metal braces to modern clear aligners. The difference is just how the pressure is applied.
For aligners, each tray is molded slightly differently from your current tooth position. When you snap it on, the plastic is fighting against your teeth trying to return to their shape. Your teeth are the ones that move. It's a slow, millimeter-by-millimeter process — but consistent enough that over 3–9 months, the cumulative movement is dramatic.
How aligners differ from braces
Metal braces apply force at single anchor points — each bracket bonded to each tooth, connected by a wire that pulls. Clear aligners distribute pressure across the entire tooth surface at once. For most minor-to-moderate cases, this is actually gentler and more precise.
A few other real-world differences:
Visibility. Braces are visible. Aligners are virtually invisible at conversation distance — most people don't notice them.
Removability. Braces are permanent fixtures for 18–36 months. Aligners come out for meals, brushing, photos, and the occasional important meeting. This is both the biggest advantage and the biggest risk — you have to actually wear them.

A realistic treatment timeline
Here's what a typical Hello My Teeth day-time plan looks like, week by week:
Weeks 1–2: You get your impression kit or 3D scan. Your impressions ship to our lab and our dentists review them to design your custom plan.
Weeks 2–3: You receive your 3D treatment preview — a visual simulation of exactly how your teeth will move. You approve it before anything is manufactured.
Weeks 3–4: Your full set of custom aligners arrives, numbered and labeled in sequence.
Months 1–2: Initial tenderness is normal for the first 2–3 days of each new tray. Small visible changes start appearing by week four.
Months 3–6: Major visible progress. Most day-time patients finish treatment in this window.
Post-treatment: Retainers for life. The first six months, you wear them nightly. After that, 3–4 nights a week is typically enough to maintain results.
What aligners can't fix
This is where honesty matters. Clear aligners are genuinely excellent for most cosmetic alignment issues — but they're not a cure-all. Here's what they typically can't handle:
Severe skeletal issues. If your upper and lower jaws are significantly misaligned (not just your teeth), you're likely looking at jaw surgery or traditional orthodontics with jaw-expansion devices.
Major bite corrections. Deep overbites, severe underbites, and significant crossbites usually require fixed appliances that aligners can't replicate.
Large vertical tooth movement. Pulling a tooth significantly up or down (intrusion or extrusion) is harder with aligners than with braces.
Rotating severely twisted teeth. Small rotations are fine. Teeth rotated 20+ degrees may need attachments or traditional braces.
